Failure Mode Effect Analysis
Failure Mode and Effect Analysis (FMEA) is a flexible, powerful and modular tool to asses risks and to develop mitigation on any ground of discipline, for any type of system, components, equipment and functionality. Nowadays the FMEA study is increasingly required by the rules in force.
The FMEA study aims to investigate whether the effects of single component failure, common mode failures or hidden failure could lead to critical situations which may jeopardize the safety of the vessel or the environment or to loss of production.
We provide complete FMEA packages, comprehensive of Test Programme and relevant witnessing of Proving Sea Trials, for a profuse scope of investigation:
- Spread Mooring Systems,
- Pipe Laying Systems,
- Cable Laying Systems,
- Lifting Equipment,
- Anti-heeling System
- and, last but not least, Dynamic Positioning.
FMEA for Dynamic Positioning
The most of our provided FMEA services are largely issued for DP Vessels, where we have a proven background of superintendence, design, building and management; our deliverables are approved by the applied Classification Society (e.g. ABS, DNV-GL, RINA, RMRS, LR, etc.).
We are IMCA Member since 2010 and we pay all our efforts to raise at the highest and affordable recommended standard for our studied fleet.
Medea includes in the team DP operators, mariners and engineers, all with considerable expertise in preparation of FMEA.
Based on our consolidated team and experience, Medea is able to offers a full range of services on the ground of Dynamic Positioning, to ensure a vessel or mobile offshore units is kept within its specified & designed position and heading limits.
DP services and packages include, but not limited to, the following options:
Failure Mode and Effect Analyses for both new build and existing DP vessels
GAP Analysis & Technical Adivse for upgrade works
Technical review of vessel design specification to meet any DP Standard
Annual DP trials, DP Annual Audits
DP Capability analyses, including HAZIDs
Consultancy in DP incident investigation& disputes for DP vessels
Provision of ship-specific ASOG (Activity specific operating guidance), CAM (Critical activity Mode – formerly called Safest Mode of Operation) and TAM (Task Appropriate Mode)
Preparation of DP Operations Manuals
Remote DP Annual Audit & “BeReady” software.
